Minor Ailments
- First, select the day for your appointment. If the message says “Please select another day.” then the day you have requested is not available.
- Select the time for your appointment. If the time is greyed out it is not available.
- Complete the personal information section. Required fields are marked with an asterisk (*).
- Click submit.
DO NOT include any personal details like health card number, etc. in the description field.
